Resident Experience Coordinator - Social Worker - BSW - Full-Time - Days - Residential Living Facility - ACH

Essential Duties and Responsibilities: 

Business Office Functions:

  • Receive and respond to referral inquiries.  Maintain future referral spreadsheet and/or wait list if appropriate

  • Meet with prospective residents/families to discuss possible residency at RLC either at RLC or in the resident home

  • As the RLC liaison, assist with effective transitions in care (i.e. from swing bed to NF, NF to home with HH, etc.) and network with SL and neighboring facilities regarding current census levels and admission capacity.

  • Meet with potential residents in SL or regional facilities to discuss potential residency and answer questions

  • Maintain daily census records.

  • Code all accounts payable items via Workday.

  • Performs payroll corrections and entry for DON sign off.

  • Manages care assessment calendar to ensure they are done in a timely manner.

  • Maintain confidentiality regarding residents, families, staff, and the organization.

  • Review resident complaints & grievances, make necessary oral/written reports as appropriate w/in required time frame.

  • Maintain policy review database, circulate for review/edits.  Submit new policies for review/approval.

  • Develop and maintain good working relationships with other facility departments within Saint Luke’s, and other health related facilities and organizations.

  • Report all incidents/accidents in accordance with established System Risk Management policies.

  • Attend and participate in in-service training programs. Working with the DON, assist in program development and tracking.

  • Participate in fire, safety, and disaster preparedness drills

  • Possess the ability to deal tactfully with personnel, residents, family members, government agencies and their representatives, and the general public.

  • Must have patience, tact, enthusiasm and a cheerful disposition.

1 year

Licensed Clinical Social Worker (KS) - Kansas Behavioral Science Regulatory Board

Bachelor's Degree - Social Work
