The Opportunity: 


Anderson County Residential Living Center is part of the Saint Luke’s Health System and is located in Garnett, KS. This top performing facility is hiring for a Supervisor Nursing ADON to join the Long Term Care - Residential Living Center. This position serves as the Assistant Director of Nursing for the ACH Long Term Care Unit (RLC). 

 

This unique residential living center is connected to our Anderson County Hospital and clinics allowing residents and staff quick access to additional care when needed. Quality care for residences and their families is a key priority for this location and this role.

 

The Work: 

  • Accountable for the development, implementation, and evaluation of the care provided on the Residential Living Center consistent with the established strategic plan goals and objectives for patient-centered care.
  • Works in collaboration with organizational leadership to identify areas of improvement regarding practice, policies, and processes.
  • Supports optimal care that requires specialized knowledge, critical thinking, and skills based on principles of psychological, social, physical and biological sciences that utilize nursing processes.
  • Accountable for staff scheduling, weekly IDT meetings, charge audits, leads actions pertaining to interaction or investigations for resident concerns, regulatory compliance, and clinical policies and procedures.
  • Assists with interacting and coaching staff, recruitment, PMP, staff education, resident rounds, quality improvement and PIP teams, PEAK, patient resident screening and interview, new services/program development, and community engagement.
  • Assists in employee engagement and retention. Collaborates with multi-disciplinary team to exceed customer expectations in a resident-centered and healing environment.
  • Serves as the Assistant Director of Nursing for the ACH Long Term Care Unit (RLC).
  • Models the SLHS leadership competencies and core values including professionalism, innovation, adaptability, independent judgment and solid decision making. Takes call for Residential Living Center.
  • Takes call for Residential Living Center.
  • Receive and respond to referral inquiries.  Maintain future referral spreadsheet and/or wait list if appropriate.
  • Meet with prospective residents/families to discuss possible residency at RLC either at RLC or in the resident home.
  • Maintain confidentiality regarding residents, families, staff, and the organization.
  • Develop and maintain good working relationships with other facility departments, departments within the Saint Luke’s, and other health related facilities and organizations.
  • As the RLC liaison, assist with effective transitions in care (i.e. from swing bed to NF, NF to home with HH, etc.) and network with SL and neighboring facilities regarding current census levels and admission capacity. 
  • Maintain daily census records. 
  • Performs payroll corrections and entry for DON sign off.
  • Manages care assessment calendar to ensure they are done in a timely manner. 
  • Participate in discharge planning, development and implementation of social care plans and resident assessments. Assist in interpreting social, psychological, and emotional needs of the residents/family to the medical staff, attending physician, and other resident care team members. Prior to the care plan meetings, SS Rep. will send out notice/invite letters to family. 
  • Assist in making appointments for the residents/family as requested and appropriate and coordinate transportation to other facilities, residents' home, off premise physicians, and ancillary outpatient services. 
  • Attend and participate in in-service training programs. Working with the DON, assist in program development and tracking.

This is an unpredictable position that will require you to change priorities quickly and often. Strong ability to prioritize tasks and multi-task is required. Must have patience, tact, enthusiasm and a cheerful disposition.
