POSITION SUMMARY
The Physician Assistant/Nurse Practitioner provides primary health care services including acute, chronic, and preventative medical care, with an emphasis on chronic disease management and pediatric care using the Patient Centered Medical Home model of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides diagnostic and therapeutic services to patients including performing history and physical exams, prescribing medications, managing acute, chronic, and preventative care, making referrals for hospital and specialty services when applicable under the direction of a collaborating physician
- Reviews and interprets received laboratory, radiology, and consultation reports in a timely manner and provides direction to nursing staff or directly contacts the patient with results and/or treatment recommendations
- Provides quality care to patients by following the most current evidence-based guidelines, best practices, and state, local, and federal guidelines
- Provides ongoing patient education regarding current diagnosis and plan of care
- Assists in establishing standards of care for management of patients, in collaboration with other providers and staff
- Provides accurate documentation regarding the reason of the visit, medical history, current medication list, recommended plan of care, orders for medications and/or labs, referrals, transition of care, and follow-up
- Identifies barriers to patient care and delivery of services and assists with formulating the plan of care and coordination of care by making referrals to internal support services or outside collaborating organizations
- Collaborates with the Chief Medical Officer (CMO), Chief Clinical Officer (CCO), other providers, and other staff to evaluate services as well as plan and develop programs
- Participates and provides input in monthly Medical Executive meetings and other committee meetings as well as participating in Quality Improvement (QI) activities, Peer reviews, Pharmacy and Therapeutics, and evaluation and implementation of clinical programs
- Reviews current literature to remain current with advances in therapeutic interventions
- Ensures participation in continuing education requirements
- Maintains patient confidentiality at all times in accordance with HIPAA regulations
MINIMUM REQUIREMENTS
- Graduated from an accredited Nurse Practitioner or Physician Assistant program
- Spanish bilingual required
- Licensed in the State of Missouri
- Basic Life Support (BLS) Certification
PREFERRED REQUIREMENTS
- Active Missouri DEA and BNDD Licensure
- Two years' experience as a PA or Nurse Practitioner in an Federally Qualified Health Center
- Experience with eClinical Works (eCW)
