POSITION SUMMARY
The Nursing Supervisor at KC CARE is responsible for overseeing the daily operations of the clinical nursing team while ensuring that high-quality, patient-centered care is delivered in line with KC CARE's culture code and mission. This position combines leadership, direct clinical care, care coordination, and performance management to ensure that both clinical and operational goals are met. The Nursing Supervisor will play a key role in the development, implementation, and evaluation of nursing programs, policies, and practices, including those related to primary care, specialty care, and HIV services. This position works collaboratively with other leaders to integrate clinical services with all health center programs.
 
 
ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide direct patient care, including conducting assessments, developing treatment plans, and delivering nursing interventions. 
  • Maintain accurate and timely documentation of patient care in the Electronic Health Record (EHR) system. 
  • Coordinate and monitor patient care activities, ensuring continuity and quality of care. 
  • Serve as a primary point of contact for patients and families, facilitating communication and addressing concerns. 
  • Manages medication alerts, hospital admissions, and ER visits 
  • Educate patients and their families about health conditions, treatment plans, medication management, and self-care strategies. 
  • Conduct comprehensive assessments of patients' healthcare needs including medical, social, and behavioral aspects. 
  • Monitor patients' progress and adjust care plans as necessary to achieve optimal health outcomes. 
  • Coordinate referrals to specialists, community resources, and other healthcare providers. 
  • Provide specialized patient education as needed in an individual or group setting. 
  • Provide individual provider nurse care support for at least one provider, if indicated. 
  • Collaborates with providers to ensure timely and effective delivery of immunizations and treatments. 
  • Ensure the delivery of high-quality, patient-centered care across all clinical departments. 
  • Develop and monitor policies, procedures, and protocols to ensure the highest standards of care that meets all state and federal guidelines.   
  • Manage clinical budgets, resources, and staffing to optimize operational efficiency. 
  • Monitor adherence to the PCMH model of care (patient centered medical home) and develop supporting policies and procedures. 
  • Provide direct supervision, mentorship, and support to nursing staff, ensuring adherence to clinical protocols and best practices. 
  • Manages daily and long-term staff coverage, including reviewing and approving time off requests, monitoring call-ins and ensuring adherence to attendance expectations in alignment with KC CARE policy
  • Ensure compliance with all relevant healthcare regulations and accreditation standards. 
  • Monitor and evaluate clinical performance metrics, patient satisfaction, and health outcomes. 
  • Lead quality improvement initiatives to enhance patient care and safety. 
  • Ensure adherence to evidence-based practices and clinical guidelines. 
  • Participate in accreditation and compliance activities, including audits and inspections. 
  • Represent the organization in external meetings and forums related to clinical operations and healthcare delivery. 
  • Participate in community health fairs, screenings, and educational events. 
  • Coordinates vaccine and medication management, ensuring proper storage, handling, and administration in accordance with established protocols and guidelines
  • Manage and/or participate in health center programs such as Show Me Healthy Women, Vaccines for Children, Ryan White, Infection Control, STD disease monitoring, Primary Care Home Health, or specific disease monitoring and impact such as diabetes, hypertension, hepatitis C or HIV, etc. 
 
 
MINIMUM REQUIREMENTS
  • B.S.N. in Nursing  
  • Licensed as a Registered Nurse in the State of Missouri or ability to obtain.
  • 4 years' experience in internal medicine/family medicine/community health 
  • 2 years' progressive supervisor experience, oversight and evaluation and budget responsibilities.  
 
PREFERRED REQUIREMENTS
·        Straight Connector 1864195090, Shape7 years of clinical experience, with at least 3 years in a leadership or supervisory role. 
·        Strong knowledge of healthcare regulations, quality assurance, and clinical best practices. 
·        M.S.N. in nursing or related field  
·        Experience working in a community health or FQHC setting. 
·        Experience in Federal grant management
